  • Repeat capture conditions once in header and once before graphics.
  • Use consistent heading verbs across therapists.

Write for three audiences without three separate PDFs if possible

Layer content: executive summary for all, detail sections for clinicians, patient-friendly glossary footnotes where jargon appears. If you must split versions, version-control them ruthlessly.

Measurement blocks need captions, not orphan numbers

Every table should state units, landmark definitions, and date. Cross-link to images when helpful. Insurers reject mystery spreadsheets.
